The Schengen visa application form must be filled out only in Latin letters in Russian (transliterated), English, or the language of the consulate where the visa is being issued. To avoid confusion, indicate the name that is stated in your birth certificate. In this item, do not write "g." or "gor." before the city name. The German Consulate asks to indicate the place of birth in the German variant; to assist applicants, there is a special table that can be found on the consulate's website. If your city is not on the list, use the transliteration table. 6. Country of birth - if you were born in the USSR before '91, you should indicate USSR / СССР. In online application forms, the country USSR is usually not available; in this case, you should indicate the country's current name. 7. Current nationality - Russian Federation / Российская федерация If you were born before 1991, indicating USSR nationality is not mandatory; leave this field blank. 17. Applicant's home address and email address - in this item, indicate your actual place of residence. Students indicate the name of the educational institution and its address.Abbreviations like ООО, ОАО, ЗАО, ОДО, etc., do not need to be deciphered or translated into another language. Write: ООО, ОАО, ZAO, ODO, etc.
Company names also should not be translated into English; write them in transliteration. For example, if you work for OOO "Zagorodny Dom", you should write OOO Zagorodny Dom in the Schengen application, not Ltd. Country House, etc.
21. Main purpose(s) of the journey - Tourism 22. 24. Number of entries requested - single or multiple entry 25. Duration of stay - indicate how many days in total you will spend in the Schengen area.For example, you are going to Prague for 5 days, and you chose a car as transport, the leisurely journey there and back will take another 3-4 days. Therefore, in item No. 25, you indicate 9 days (5 in Prague + 4 on the road).
26. Schengen visas issued in the last three years - indicate the validity period of the visas. The forms provide space for only one visa; in this case, the last visa is indicated. Others can be written in by hand (if there is space) or not indicated at all. In any case, if they existed, you must provide copies of them. 27. Fingerprints, provided previously when applying for a Schengen visa - if your fingerprints were taken when applying for previous Schengen visas, indicate this in this item. 28. Entry permit for the country of final destination, if required - this item is filled out only when obtaining a visa for transit to a third country. 29. Intended date of entry into the Schengen area - the start date of your Schengen visa. Please note that it should not be later than the day you are supposed to cross the Schengen border. 30. Intended date of departure from the Schengen area - the expiry date of the visa. 31. Surname(s), first name(s) of the inviting person(s) in the Schengen Member State(s).
